PATIENT CARE LIAISON

Seeking highly-motivated, self-starter to develop referral sources and coordinate patient care along with the physician, facility, and our care team in OAKMONT and surrounding areas. Qualified candidate is a PA-licensed LPN or RN with proven success in the coordination of Hospice and Home Health services in the advertised area.  This full-time position offers an attractive pay rate; incentive plan; compensation package - including Highmark Medical/Dental/Vision; cell phone; mileage reimbursement; 401k Plan; and paid days off.  Send resume to:
